A. Event Details

The above-named youth has been invited to participate as a singer in the Emerging Artist program in the Pro Coro Canada Good Friday concert. The event details are as follows:

B. Consent for Participation

I give my informed consent for the above-named participant to take part in the rehearsal(s) and concert described above, and any reasonably related activities occurring at the venue in connection with the events.

I understand that the rehearsal on March 20 may end at approximately 9:30 PM, and that on April 3 the participant will be at the venue from approximately 6:00 PM until the conclusion of the concert (approximately 9:30 PM or later).

C. Supervision & Youth Protection

Pro Coro Canada will take reasonable steps to provide a safe environment for youth participants. To that end, the following safeguards will be in place during all rehearsals and the concert:

1. Two-Adult Rule. Pro Coro Canada will endeavour to ensure that at least two unrelated adults (staff, professional singers, or designated volunteers) are present when youth participants are in its care.

2. Visibility. Adults working individually with a youth participant will remain in open sight of other adults. No one-on-one situations will occur behind closed doors.

3. Defined Period of Care. Pro Coro Canada's duty of care for youth participants begins at sign-in upon arrival at the venue and ends at sign-out at the conclusion of the rehearsal or the concert. The participant is not under Pro Coro Canada's supervision during transit to or from the venue. Responsibility for the participant remains with the parent/guardian until sign-in and resumes upon sign-out.

4. Code of Conduct. All Pro Coro Canada staff, professional singers, and volunteers interacting with youth participants are expected to always maintain appropriate professional boundaries.

I acknowledge and understand the safeguards described above. I understand that I may contact the Managing & Artistic Director at any time with questions or concerns regarding youth safety.

D. Transportation

I understand that Pro Coro Canada does not provide transportation to or from rehearsals or the concert, and that Pro Coro Canada's duty of care does not extend to the participant's transit to or from the venue.

Where the participant travels with another parent, guardian, or designated driver, that person acts independently and is not an employee, agent, or representative of Pro Coro Canada. Pro Coro Canada does not verify the licensing, insurance, or driving record of any third-party drivers.

I acknowledge the inherent risks of the selected mode(s) of transportation and confirm that I have discussed safe travel expectations with the participant. Should transportation arrangements change, I will notify Pro Coro Canada in advance.

E. Acknowledgment of Risk

I acknowledge that participation in choral rehearsals and performances involves certain inherent and unavoidable risks, including but not limited to: physical exertion, slips or falls, interaction with members of the public, travel to and from the venue, exposure to weather conditions during travel, and risks associated with being in a public venue during evening hours.

I freely and voluntarily assume these risks on behalf of the participant and understand that Pro Coro Canada, its staff, volunteers, and the venue will take reasonable precautions to ensure participant safety but cannot eliminate all risk.

F. Medical Authorization

In the event of a medical emergency, I authorise Pro Coro Canada staff or designated adults to seek and obtain emergency medical assistance and, where necessary, to consent to medical treatment - including ambulance transport - for the above-named youth if I cannot be reached. I acknowledge that reasonable efforts will be made to contact the parent/guardian prior to treatment, where practicable. I understand that I will be financially responsible for any medical costs incurred.

G. Behavioral Expectations

The participant takes ownership and responsibility for their conduct, arrives prepared and on time, follows all directions from Pro Coro Canada staff and venue personnel, treats all persons and property with respect, keeps electronic devices put away during rehearsals and the concert, and always represents Pro Coro Canada with professionalism.

I confirm that the participant has been informed and understands the expectation. I understand that failure to comply may result in removal from the rehearsal or performance. I understand that I may be contacted and asked to arrange for the transport of the participant.

Parents or guardians who choose to attend rehearsals or the concert must respect the rehearsal and performance environment and the direction of Pro Coro Canada staff and volunteers. Parents and guardians are asked not to interrupt rehearsals, give instructions to participants, or otherwise interfere with the supervision and instruction of youth. Pro Coro Canada reserves the right to ask any attendee to leave the rehearsal or performance area if their conduct is disruptive to participants or staff.

H. Standard of Dress

To present a professional appearance in concert in-person and on screen, all concert clothing must be of a respectable quality, clean, and not faded. Appropriate personal hygiene shall be maintained for all rehearsals and performances, with discreet use of perfumes, deodorant, or cologne.

(1) Concert Black - a choice of the following:

  1. Black suit, matching jacket/pants, and with black collar shirt with a solid black tie
  2. Black pants/dress slacks - no denim, no yoga pants, no sportswear
  3. Black blouse (3/4 sleeves at least), no t-shirt or tank-top
  4. Black formal jacket
  5. Black skirt or dress (3/4 sleeves at least) to the knee or below - tea length preferable

(2) In addition:

  1. Black socks, nylons, or tights
  2. Black belt, black suspenders, if worn
  3. Black, traditional, polished shoes matching attire, no open toes, no sport shoes
  4. Simple, decorative jewellery and hair accessory, no large/dangly earrings (not touching shoulder), and discreet necklace
  5. Items for personal expression must be worn under a layer
  6. Black folder, double, to accommodate paper scores or tablets for the performance

I confirm that I have reviewed the dress code above with the participant and will ensure they are appropriately attired for the concert.
